Title: 慢性腎衰竭病人接受血液透析時的眼壓變化
Intraocular Pressure Fluctuation During Hemodialysis in Chronic Renal Failure Patients

Abstract: 目的:探討慢性腎衰竭病人接受血液透析當中眼壓變化的趨勢,並同步觀測血壓及眼前房深度之變化。
病人及方法:本研究包括43個慢性腎衰竭病人的86隻眼睛。在徵求書面同意之後,我們於病人接受血液透析前、及開始後的每一小時,以筆型眼壓計測量病人的臥姿眼壓,並同步以血壓計測量臥姿右手臂血壓,及以生體超音波A-scan測量眼前房之深度。總共測量五次。
結果:我們觀測到在接受血液透析之中及結束時,眼壓表現出明顯之變化。這些時間點的眼壓變化和透析前比較皆有顯著意義的不同(p=0.0068)。在血液透析開始的一小時,眼壓有明顯的上升,之後則緩緩下降,直到血液透析結束再有一升高的現象。血壓在這五個時間點則未見顯著有意義的變化。血液透析當中眼壓的趨勢走向,也沒有和血壓或前房深度之變化有相關。
結論:在血液透析當中,我們觀測到眼壓之獨特變化。這眼壓的波動引起我們關心它對血液透析病人之影響,如青光眼病患。在整個血液透析當中,血壓及眼前房深度之變化和眼壓變動並無相關。
Purpose: To study the trend of intraocular pressure (IOP) changes in the uremic patients during hemodialysis and their relationship with blood pressure and anterior chamber depth changes.
Patients and Methods: We completed the examinations in 86 eyes of 43 chronic renal failure patients. After obtaining the informed consent, the IOP was measured by a tono-pen and anterior chamber depth by A-scan ultrasonography. Blood pressure and all other data were recorded in a supine position before hemodialysis and every hour since then for four hours..
Results: Triphasic TOP changes were noted during hemodialysis. Significant difference was noted among the five TOP measurements (p=0.0068). It showed an initial elevation one hour after hemodialysis, and with a gradually depression trend throughout the hemodialysis, and then elevated again immediately after the hemodialysis. There were no significant changes of the blood pressure and anterior chamber depth changes in the whole procedure of hemodialysis. There was also no correlation among IOP, mean arterial pressure, and anterior chamber depth during the hemodialysis.
Conclusion: We observed a triphasic IOP changes during hemodialysis. No correlation was found between mean arterial blood pressure and anterior chamber depth changes during the course of hemodialysis.
